Please read below before filling out the form.

Policy:
You must be 18 years or older to be tattooed.
​You are required to present a valid government issued photo ID at the time of your appointment. You will not be tattooed without one.
If you are 30 minutes or more late to your appointment, without prior notification to your artist, you will forfeit your appointment as well as your deposit. You will need to rebook with a new deposit. If you are an hour or more late to your appointment, with or without notification, you will also forfeit your appointment as well as your deposit.
ALL appointments require a nonrefundable, nontransferable deposit. Even if you have booked with me before, it is still required. Your deposit amount is dependent on your tattoo. You are allowed one reschedule, for emergency purposes only. After which, if you need be rescheduled more than once you will be required to leave 50% of your tattoo down as a deposit. If you no call, no show, you will not be rebooked.
I have the right to refuse any appointment, without given reason.
I do not tattoo genital/nether regions, I will not respond to you if you ask.
Any changes with your tattoo/tattoo design may result in additional fees.
Full day sessions are for one person only, you cannot book a full day for several people.
I do not copy others original artwork. Do not inquire if you are looking for someone else's specific artwork as it goes against copyright laws.
You are only allowed one guest. Children are not allowed in the shop.
If you are sick you will need to reschedule. Sickness will hinder your bodies ability to heal your tattoo.
I only accept cash, or card for an additional charge.

Please note this does not schedule you immediately, it is to inquire about a new appointment. A nonrefundable, non-transferrable is required to book an appointment. Please note it may take more than a day to respond due to high volume. Sunday appointments are not available. You cannot be tattooed if you are pregnant, breast feeding, or are on certain medications. You cannot go swimming after a tattoo.

For coverups you will be required to come for an in-person consult.
